Srotas Health is an AI healthcare technology company building SrotasIQ, a next-generation platform for clinical trials, patient engagement, and AI-driven workflows. We work closely with research sites, clinicians, and sponsors across the UK and US to reduce recruitment delays and improve patient outcomes using AI.
Role Overview
We are looking for a Backend Developer to help design, build, and scale core backend services for our multi-tenant healthcare platform. You will work on APIs, event-driven workflows, integrations (voice, SMS, email), and secure data pipelines that power real-world clinical operations.
This is a hands-on engineering role with real production impact.
What you’ll do
- Build and scale backend services using Node.js, TypeScript, NestJS, and Python.
- Design APIs, data models, asynchronous workflows, and integrations.
- Solve system-design challenges around scale, reliability, security, and performance.
- Build integrations with clinical systems, communication platforms, calendars, and third-party APIs.
- Work on background jobs, queues, workflow orchestration, retries, and event-driven systems.
- Improve database performance, caching, API latency, and platform reliability.
- Contribute to our cloud architecture across Azure and GCP.
- Work with Cloudflare for security, traffic management, caching, and edge capabilities.
- Partner with AI and product teams to ship features end-to-end.
- Review code, improve engineering standards, and mentor engineers as the team grows.

What we’re looking for
- 6+ years of backend engineering experience.
- Strong hands-on experience with Node.js, TypeScript, and NestJS.
- Good experience with Python for backend services, workers, or data workflows.
- Strong system design and software architecture skills.
- Experience building scalable APIs and distributed backend systems.
- Experience with queues, asynchronous processing, background jobs, and event-driven workflows.
- Strong understanding of databases, data modelling, caching, and performance optimisation.
- Experience deploying and operating applications on Azure and/or GCP.
- Familiarity with Docker, CI/CD, cloud security, observability, and production troubleshooting.
- Experience with Cloudflare is highly preferred.
- A product mindset and willingness to take ownership from design through production.

Nice to have
- Experience with healthcare, clinical research, EHR/EMR integrations, FHIR, or HL7.
- Experience with AI-enabled, data-heavy, or regulated products.
- Familiarity with Temporal, Kafka, RabbitMQ, BullMQ, or similar workflow/queueing tools.
- Experience with Azure App Service, Azure Container Apps, Cosmos DB, Cloud Run, Pub/Sub, BigQuery, or Kubernetes.
