Senior Care Assistant Rivermere Care Home – Sevenoaks

We’re now seeking a warm, enthusiastic, and dedicated individual to join our award-winning team as a Senior Care Assistant at Rivermere Care Home in Sevenoaks.
About The Role
As a Senior Care Assistant, your role is pivotal in delivering outstanding care:
- Work alongside the care team to provide high standards of personal care, fostering an environment that is safe, innovative, inclusive, and supportive for residents, visitors, and staff.
- Monitor resident wellbeing, spot key changes affecting care needs, and update care plans accurately. Ensure proactive adjustments.
- Collaborate in person-centred care planning:
- Implement care strategies.
- Conduct monthly evaluations of effectiveness.
- Support residents with their medication requirements, ensuring correctness and compliance.
- Supervise and mentor staff members, assisting their professional growth.
- Serve as the primary point of contact for:
- Residents and their families/visitors.
- Health and social care professionals.
- Addressing reasonable requests and escalating concerns/complaints.

Key Responsibilities

Resident Care Coordination:
- Monitor daily changes in resident health, behaviour, and comfort.
- Update records and amend care plans reflection real-time needs.

Person-Centric Evaluations:
- Implement approved care plans consistently.
- Schedule and carry out monthly evaluations with documentation.

Medication Support:
- Assist residents with scheduled medication, as delegated.
- Ensure accuracy and adherence to professional guidance.

Staff Supervision & Development:
- Conduct one-on-one staff supervision as required.
- Foster a collaborative culture through peer support and mentorship.

Relationships & Communication Hub:
- Liaise with residents, relatives, visitors, and external parties (e.g., healthcare professionals).
- Facilitate information-sharing while escalating serious concerns.
About You: What We’re Looking For

Qualifications:
- NVQ Level 3 in Care (or equivalent) tailored for care homes.
- Validated cover in healthcare, social care, or similar sector role (minimum 2 years).

Skills & Attributes:
- Excellent verbal and written communication to cater diverse audiences.
- Positive attitude – compassionate towards residents and proactive with staff.
- Flexibility to adapt to varied needs, including covering multiple responsibilities.

Essential Pre-Requisites
The role requires:
- A DBS Disclosure check (cost covered by Avery Healthcare).
