Senior Clinical Practitioner Band 7 to 8a Progression Post

Senior Clinical Practitioner – Hospital/Community Specialist Palliative Care

Community Services Division

We are offering an exciting opportunity for an experienced and highly motivated clinician to join our Specialist Palliative Care Service in a progression role (Band 7 to Band 8a).

This role emphasises preventing avoidable hospital admissions by delivering proactive, responsive and skilled assessments within hospital and community settings. The aim is to enable patients to either reduce their length of stay in hospital or remain safely in their own homes for as long as possible.

Key Responsibilities

The post holder will:

  • Provide specialist palliative care assessments across hospital and community settings.
  • Work at Band 7 initially, with progression to Band 8a based on achievement of defined competencies and service expectations.
  • Combine advanced clinical practice with leadership and service development, focusing on:
    • Early discharge
    • Admission avoidance
    • Reducing reliance on acute hospital services
  • Identify patients at risk of deterioration, intervene early, and coordinate care to manage:
    • Complex symptoms
    • Psychosocial needs
    • Anticipatory planning and clinical decision-making
  • Prevent crisis-driven admissions to hospital and facilitate timely, appropriate care escalation when necessary.
