Community Nursing Team Opportunity
We have an exciting opportunity with the Community Nursing team. We have opportunities throughout the Fylde coast which will be discussed at interview.
We have Full & Part Time positions available as well as Permanent & Fixed Term.
Requirements
Interested applicants will need to be registered nurses who have relevant nursing qualifications to degree level, specialist post registration education and leadership training or equivalent experience. The post holder requires at least 18 month's experience at Band 5.

Responsibilities
The successful candidate will be able to:
- Assess, plan, implement and evaluate care for a defined district nursing caseload
- Ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team
- Work in partnership with the primary health care team, liaise and cooperate with everyone involved in holistic patient care
- Demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals
- Provide clinical supervision to junior staff and support staff development
