Band 6 Nurse / Paramedic – Hospital at Home (H@H) Service

Urgent Community Response (UCR) Team – Hillingdon Borough

About the Role

This exciting position offers a unique opportunity to join Clarke Nottinghamshire and Leeds Partnership NHS Foundation Trust (CNWL) as a Band 6 Nurse or Paramedic, delivering hospital-level care in the patient’s own home. As part of our innovative Hospital at Home (H@H) service, you will collaborate with a multidisciplinary team (MDT) to prevent unnecessary emergency admissions and support early discharge—enhancing patient well-being, safety, and dignity.

Key Responsibilities

Clinical Care Delivery

  • Conduct comprehensive clinical assessments in the patient’s home environment for those at risk of hospital admission.
  • Deliver evidence-based, patient-specific interventions aligned with NICE guidelines and CNWL protocols.
  • Manage a defined caseload of complex, acute, and frail patients, making judgement-based clinical decisions while escalating to senior clinicians when required.
  • Contribute to daily MDT reviews, advocating for optimal patient care pathways.
  • Monitor and evaluate care plans, ensuring safe and timely discharge to either primary care, specialist services, or further care pathways.

Technical Competencies

Demonstrate proficiency in:

  • Independent home-based assessments, including frailty screening and stabilisation.
  • Use of digital monitoring tools (e.g., remote vital signs, alert systems).
  • Point-of-care testing (e.g., blood glucose, ECG, wound assessment).
  • Cannulation, IV therapy administration, catheter care (decanting, emptying, and management).
  • Following selective de-escalation protocols to ensure appropriate upscaling pathways.

Collaboration & Care Coordination

  • Partner closely with GPs, acute hospital teams, pharmacy, social care, and community nursing to ensure a seamless, collaborative approach.
  • Maintain open communication with achieves, nurses, physicians, and allied health professionals.
  • Work flexibly to relieve colleagues and rotate across boroughs as required by the service demand.

Mentorship & Professional Development

  • Provide informal and formal line management support to colleagues.
  • Stay current with primary and secondary healthcare developments, incorporating best practices into patient care.
  • Uphold CNWL’s governance standards, quality assurance frameworks, and SCARF principles (Safe, Accountable, Reflective, Flexible).
  • Uphold NMC standards for nursing conduct, performance, and ethics.

Patient & Carer Support

  • Offer health education, guidance, and emotional support to patients and carers.
  • Foster strong rapport and interpersonal skills in a diverse, multi-cultural setting.

Essential Criteria

✅ Current registration with the Nursing and Midwifery Council (NMC). ✅ Bachelor’s degree (or equivalent) or expected completion date. ✅ Experience in case management and coordinating acute care plans—preferably in Paramedic, Urgent Care Centres (UCC), ITU, AMU, MAU, CCU, or A&E settings. ✅ Proven acute disease management, crisis care, and the ability to direct/provide patient care independently and collaboratively. ✅ Familiarity with community-based nursing.

Desirable Criteria

🔹 Teaching or mentorship qualifications (e.g., ENB 998 or an equivalent workplace assessor qualification). 🔹 Experience supporting frail patients with complex care needs. 🔹 Demonstrated ability to educate multi-disciplinary teams in disparities courses.
