The Team & Environment
The Sirona Integrated Neighbourhood Teams (INT) provide innovative, active rehabilitation to patients in their place of residence, helping them maximise functional potential and remain independent in their homes. Our services include:
- Supporting hospital discharges & admission avoidance
- Assisting with long-term self-management
- Enabling patients to live better lives in their home environment
You’ll work within a friendly and supportive team, alongside:
- Therapists
- Assistant Practitioners
- Nurses & Nurse Associates
- Advanced Clinical Practitioners
- Support Workers
- Administrators

Key Responsibilities
Assessment & Care Delivery
- Conduct initial holistic assessments and implement/evaluate care programmes for both acute and chronic patients (often with complex needs)
- Apply clinical reasoning skills to deliver evidence-based, patient-centred interventions, ensuring high-quality and harm-free care
Service & Caseload Management
- Manage proactive service delivery and own caseloads efficiently
- Allocate work and coordinate team arrangement to maximise patient care
Collaboration & Discharge Planning
- Build strong, communicative relationships with service users, carers, and families to:
- Prevent avoidable admissions
- Facilitate timely discharges
- Support achievable goals that enhance quality of life and independence
- Facilitate appropriate referrals to other services based on assessment findings
Equipment & Risk Management
- Assess and arrange equipment provision within scope of practice
- Report any equipment-related issues
- Identify risks and take appropriate action to minimise harm to self, colleagues, and service users
Team Development & Record-Keeping
- Take responsibility for developing junior staff, peers, and students
- Maintain accurate, professional, and legally compliant records
- Actively participate in training, possibly assisting in its delivery
