Senior Mental Health Liaison Practitioner

Are you a Qualified Mental Health Professional looking for an exciting opportunity to work in a team that will help you develop and achieve your goals? Oldham Liaison Mental Health is recruiting for a Senior Mental Health Liaison Practitioner.

You’ll join an innovative team thriving to deliver a high-quality service, ensuring patients receive a comprehensive assessment, onward referral, and safety-netting when they attend A&E in a mental health crisis, are admitted on wards, or detained under Section 136.

About the Role

Working within the Local Mental Health Team (LMHT), you will handle all-age referrals from:

  • A&E
  • Hospital wards
  • Police (Section 136 detentions)

You will assess these referrals, create care plans, and ensure admission, discharge, and signposting to appropriate services.

For individuals experiencing severe and enduring mental health difficulties, you will conduct:

  • Comprehensive mental health assessments
  • Risk assessments to manage threats to self/others
  • Ongoing support planning

Main Duties and Responsibilities

(Note: This list is not exhaustive.)

  • Work as part of the multi-disciplinary Mental Health Liaison team to provide:

    • Effective and efficient responses to acute trust referrals
    • Clinical assessments and interventions focused on:
      • Relapse prevention
      • Early discharge
      • Improving quality of life
  • Conduct mental health assessments for patients in the Emergency Department, including:

    • Highly developed clinical and risk assessments
    • Management planning for safety and onward care
  • Deliver support to hospital wards addressing:

    • Complex physical and mental health needs across all age groups
    • Functional and organic illnesses (including those in later life)
  • Offer training and education to acute hospital staff on:

    • Recognising mental health conditions
    • Appropriate management strategies
    • Onward referral pathways

  • Provide clinical support to police and ambulance staff to aid:

    • Pathway decision-making
  • Manage individuals under Section 136 Place of Safety assessments, including:

    • Coordination of the formal assessment process
