Senior Neighbourhood Transformation Manager
The Senior Neighbourhood Transformation Manager will play a key role in leading and delivering neighbourhood-based transformation across Nottingham City. Working across Primary Care Networks (PCNs), VCFSE and voluntary sector partners, and system partners, the postholder will support the development and implementation of Integrated Neighbourhood Teams (INTs) in line with national policy, including the NHS 10 Year Plan and the Neighbourhood Health Implementation Programme. The role will be instrumental in coordinating multi-agency transformation, improving population health outcomes, and embedding collaborative, place-based models of care. As a Wave One NNHIP Pioneer Site, the postholder will contribute to national learning and develop a working knowledge of the Neighbourhood Health Framework over time.
Responsibilities
- Lead the development and delivery of neighbourhood transformation plans across multiple PCNs.
- Support the design and implementation of Integrated Neighbourhood Teams (INTs) aligned to national policy and local priorities.
- Work collaboratively with PCNs, community services, local authority and community and voluntary sector partners to enable integrated working.
- Drive service redesign and pathway transformation to support neighbourhood-based models of care.
- Provide programme and project management expertise to deliver complex transformation initiatives.
- Develop and maintain strong stakeholder relationships at all levels across the system, building trust across organisational boundaries.
- Create the conditions for genuine collaborative working, including facilitating open and honest conversations across partners.
- Support workforce development and organisational change required to embed neighbourhood working.
- Ensure robust governance, reporting and performance monitoring of transformation programmes.
- Contribute to the delivery of Nottingham City as a Neighbourhood Health Implementation Programme site, including national learning and accountability requirements.
- Identify and mitigate risks associated with transformation delivery.
