Senior Occupational Therapist

Occupational Therapist (Band 6) – Home First Service

Moving patients safely from hospital to home is your chance to make a real difference. Join our growing multidisciplinary team as a Band 6 Occupational Therapist on our Home First Service, where you’ll shape the care that helps people recover independently at home.

About the Role

You’ll play a pivotal role in helping patients transition smoothly from hospital to their homes through occupational therapy assessments, rehabilitation, and person-centred care. By promoting independence and preventing avoidable hospital readmissions, your work will directly contribute to better patient outcomes and improved discharge planning.

As a member of our Home First Service, you’ll drive change by delivering expert interventions within the community, working collaboratively with acute hospitals, GPs, local authority colleagues, and other health/social care professionals. Supervising Band 5 OTs, students, and support staff while managing a caseload of complex patients, this role is both varied and rewarding.

Responsibilities

  • Deliver specialist occupational therapy assessments and interventions in the community to support safe and timely discharges.
  • Collaborate on discharge planning, ensuring patients leave hospital with the necessary support for independence.
  • Independently manage a complex caseload while contributing to multidisciplinary decision-making.
  • Work closely with hospitals, GPs, community teams, and local authority partners to provide integrated care.
  • Mentor and support Band 5 Occupational Therapists, students, and support staff to foster a culture of learning and excellence.
  • Champion rehabilitation and positive outcomes for patients with complex health and social care needs.

About You

We’re looking for an HPC-qualified Occupational Therapist with:

  • Proven experience in managing complex, multi-problem patients in the community.
  • Strong assessment, clinical reasoning, and communication skills.
  • A passion for person-centred care and improving patient independence.
  • Excellent teamwork abilities, with a convincing track record of collaborating across multidisciplinary teams.

Hours & Workplace

The Home First Service operates Monday to Sunday (8:00am–8:00pm), including bank holidays. Full-time staff work a flexible seven-day rota, covering:

  • Early morning shifts
  • Late evening shifts
  • Long-day breaks
