Senior Occupational Therapist AMH
Band 6 Occupational Therapist – Community Mental Health Rehabilitation Team
We’re searching for an enthusiastic Occupational Therapist (Band 6) with strong mental health expertise to join our new community mental health rehabilitation service in North Lincolnshire, Scunthorpe.
About the Role
This exciting opportunity involves contributing to our 7-day community rehabilitation service, supporting individuals closer to home with intensive recovery-focused interventions.
The Community Rehabilitation Team, working collaboratively between Rotherham, Doncaster & South Humber NHS Foundation Trust (RDaSH) and North Lincolnshire Council, brings together a multi-disciplinary team to improve quality of life through local, personalised care—aligned with the NHS Long Term Plan (2019) for Mental Health.
Main Duties & Responsibilities
- Implement occupational therapy interventions within mental health pathways to help individuals achieve recovery goals and functional stability through the full OT process (assessment → intervention → planning → evaluation).
- Conduct risk assessments, document findings, report concerns, and take appropriate action where necessary.
- Develop and monitor Individual Occupation Therapy Programmes (OTPs), evaluate outcomes, and manage discharge planning.
- Apply Model of Human Occupation (MOHO) as a core framework, including standardised MOHO assessments for pre/post therapy measures and patient-centred goal planning.
- Provide one-on-one and group education/supervision, with individualised therapy plans negotiated collaboratively with patients, carers, and colleagues.
- Support carers through education and guidance on managing needs.
- Lead multidisciplinary case reviews to discuss progress and devise safe, personalised discharge plans.
- Proficiency in referral coordination, multi-agency communication, and sensitive information sharing across services.

Key Requirements
- Current registration with the Health & Care Professions Council (HCPC) as an Occupational Therapist.
- Experience in mental health community settings, with exposure to recovery-based interventions ideally.
- Competency in:
- Assessment tools (including MOHO-based therapies).
- Risk management and documentation standards.
- Patient- and carer-led goal setting.
- Ability to work autonomously, within a multi-disciplinary team, and uphold the NHS Values and Vision.
- Commitment to continuous professional development.
