Job Description: Occupational Therapist

We are currently looking for an experienced, enthusiastic, and motivated Occupational Therapist to join our older people’s acute directorate based at The Woodlands in the Home Treatment Team.

Role Overview

As an Occupational Therapist, you will be a member of the multi-disciplinary team, which includes medical staff, nursing, psychological therapies, and others.

Using occupation as the foundation, and with an emphasis on rehabilitation, recovery, and empowerment, you will contribute to the delivery of assessments, individualised interventions, and therapy/care planning as part of the team. You will be the key worker for a defined number of patients whose primary need is for Occupational Therapy intervention.

Key Responsibilities

  • Contribute to the delivery of assessments, individualised interventions, and therapy/care planning as part of the team.
  • Take the lead on the development of recovery and occupation-focused interventions.
  • Build and refine an HTT occupational therapy pathway.
  • Work as a member of a multi-disciplinary team.
  • Assess occupational needs and provide evidence-based short-term, intensive, community-based occupational therapy interventions to complex patients experiencing acute mental health.
  • Supervise junior staff/support staff to deliver community interventions.
  • Develop links with statutory and voluntary sector community resources.
  • Ensure that ongoing occupational needs are addressed as part of a safe, effective discharge/transfer process.
