The Role
This exciting role will involve working with GPs and the primary care multidisciplinary teams to improve medicines management and deliver excellent patient care. This role will involve delivery of the pharmacotherapy service as part of the GP contract and will provide an important function in supporting patients with their medicines. This patient facing role will involve taking on a prescribing role and developing your own clinical caseload in line with Achieving Excellence strategy objectives. Applicants should be qualified independent prescribers or be working towards this qualification.
Your role will be to work as a member of the NHS GGC primary care pharmacy team. The objectives of the team are to improve the quality and cost effectiveness of prescribing and medicines use for patients across NHS Greater Glasgow and Clyde. You will work closely with GPs, other practice staff, community pharmacists, care home staff, members of the primary healthcare team and central team colleagues in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing in the Health and Social Care Partnership (HSCP). You will work within GP practices, care homes and other healthcare settings to provide individualised clinical care to patients, through clinics, to improve the safety and effectiveness of their medicines, prescribing where appropriate. You should have a sound clinical knowledge, good IT skills, excellent communication and interpersonal skills and be able to work autonomously as well as part of a team. You will work on joint projects/initiatives within the HSCP as required and promote awareness of prescribing management issues.
You must be registered with the General Pharmaceutical Council and be able to demonstrate a commitment to Continuing Professional Development. You must have significant post registration experience. You will be required to travel between GP practices and care homes to carry out the duties of this post. Experience of directly working with primary care teams and knowledge of GP systems would be advantageous. This post requires PVG Scheme Membership/Disclosure Scotland Check.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern
- Location based in Glasgow HSCP
- Full-time position (36 hours per week)
- Permanent Contract
- Shift pattern for this position is Monday to Friday only with the 36 hours spread over the 5 days, although variable hours and work patterns would be considered.

Key Responsibilities
- Responsible for provision of pharmaceutical care for identified caseload of patients in primary care sites. As an autonomous clinician using independent prescribing and clinical judgement, apply expert clinical knowledge to analyse complex patient information and make clinical decisions for optimal outcome
- Provide specialist advice involving clinical interpretation of evidence and clinical judgement to medical staff and the multidisciplinary team to inform the treatment of individual patients with complex needs. This includes advice on optimal drug regimens, avoidance of adverse drug reactions, drug interactions and contra-indications ensuring compliance with medicines legislation.
- Report and review adverse clinical incidents, identifying causes of medication errors and use expert clinical knowledge to provide advice on strategies to minimise medicine use risks.
- Carry out patient face to face consultations as part of a pharmacist led clinic or from referrals made from the GP practice Multi-disciplinary Team in order to review treatment for patients with chronic diseases or resolve a pre-identified highly complex pharmaceutical care issue.
- To undertake medication reviews for individual patients inclusive of those with co-morbidities and frailty to ensure the medicines prescribed are safe, appropriate and cost effective.
- Provide effective patient consultations including appropriate physical examinations within competence
- As part of the medication review process, changes will be documented in patients’ electronic notes and patient’s blood results will be monitored and assessed to ensure that medicines are prescribed at the most appropriate dose in order to minimise adverse side effects and maximise effectiveness.
- Ongoing relevant monitoring will also be documented and carried out as part of the follow up from medication review e.g. spirometry, peak flow, blood pressure and urinalysis
- Responding to medication requests from patients, community pharmacies, medical clinicians and other healthcare professionals, assessing appropriateness in light of medical history, current medication, relevant monitoring and generating prescriptions where appropriate.
- Provision of pharmaceutical expertise to aid the medicines reconciliation process in response to Immediate Discharge Letters, clinic letters, and other communication from secondary care
- Clarify ambiguous prescriptions to avoid medication errors and challenge the prescribing decisions of others where appropriate
- To support practices with triage through telephone consultations, telephone medication requests, handling of special requests, ensuring appropriate referral of minor ailments to community pharmacy etc
- Contribute to pharmacy and prescribing strategies that support early discharge into the community setting while ensuring continuity of pharmaceutical care across the multidisciplinary team

Minimum Requirements
- Masters Degree in Pharmacy or equivalent
- Registrant with the General Pharmaceutical Council
- Independent prescriber or working towards
- Demonstrable post-registration experience
