Role Overview
We are looking for an enthusiastic band 6 Physiotherapist to work within the Community Rehabilitation Team at Home First.
Responsibilities
The Community Rehabilitation team supports people following admission to hospital to regain their independence at home. They are supported by the Rehabilitation Assistants to complete their personal hygiene and meal tasks. A large majority of these patients have orthopaedic conditions and need assessment and treatment to improve their mobility and range of movement.
You should be personable with excellent communication skills. You will be expected to work alone and within the multi-disciplinary team. You should be able to organise and prioritise your own workload and balance this with the demands of this exciting role.
We will provide support through in-house professional forums, Mandatory training, 1:1 and peer supervision. We facilitate student education and have close working relationships with neighbouring teams in both acute Health and Social Care.
Requirements
You should be HCPC registered, have appropriate demonstrable post qualification experience of working within in rehabilitation, be a car driver and have access to a car.

To provide specialist assessment, diagnosis and treatment of patients with complex Physiotherapy problems as an autonomous practitioner within clinical speciality area. To undertake line management responsibility for staff, within the service area to provide a responsive and effective service. To actively support the Team Leader in the operational management of the area and service developments on a local and/or service-wide level. To provide clinical education and training. To have responsibility for specific projects or initiatives contributing to the overall delivery of the service within a clinical speciality area. To be responsible for providing information to the Senior Management Team
