Eligibility

Please note: at this stage, the role is open only to colleagues currently employed by the Kent and Medway Integrated Care Board (ICB) and Southeast Integrated Care Boards; and current at-risk employees within Kent and Medway NHS Trusts.

About the Role

The purpose of the Population Health Insights and Improvement Team (PHIIT) is to support the ICB in undertaking its responsibilities as a Strategic Commissioner for improving population health and reducing health inequalities across Kent and Medway. The postholder will support the Head of Analytics in setting an evidence-based, long-term population health strategy.

Working in an agile manner across the ICB MDTs and other partners, the post holder will support the Head of Analytics in understanding strategic needs of the population to inform commissioning and strategic improvement plans.

Job Description

The job description is an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by their line manager. The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the Department and the Organisation.

  • Support the Head of Analytics in the delivery of population health management approaches, focusing on insight generation, annual reporting, and collaboration with strategic commissioning teams.
  • Lead analytical initiatives and contribute to planning and performance measurement frameworks to inform population health strategies and system-wide improvement.
  • Work with system partners to support and embed approaches to population health management.
  • Liaise with primary care and other teams to support the implementation of population health initiatives.
  • Liaise with 3rd party suppliers to support the implementation of reporting systems.
  • Work collaboratively with stakeholders to develop proposals for the development of new data products that will meet user needs, including novel solutions that will enable them to work in new ways and/or take advantage of previously untapped data resources.
