We are looking for an enthusiastic, motivated, forward-thinking Senior Practitioner Social Worker to join our MH Hospital Discharge and Countywide Reviewing Team, based in Eastbourne and/or Hastings, providing a countywide service. You will work alongside lead practitioners and Multi-Disciplinary Team (MDT) partners who care as much about individual outcomes as you do.
We work with working age adults (18-70) and older adults (70+) and you would be supported by a team that cares for you, just as much as it cares for our community. We offer support to people over the age of eighteen with severe and enduring mental illness, people living with dementia and people accessing support for alcohol and substance use.
We are looking for a Senior Practitioner to work across both sides of this blended, multi-functional team. We support effective and timely hospital discharges, working across East Sussex to support people who have been admitted to hospital to seek treatment for their mental health. You will work as part of a remote partnership team with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector, together with the adult and our staff, placing the adult at the centre. You will be collaborating closely with other stakeholders, working together to support safe and timely discharges from hospital. Our Countywide Reviewing Team also supports a variety of review activity, including post-hospital, Section 117 Aftercare, those in receipt of a Direct Payment and statutory annual reviews within individuals' own homes, including supported living and care home settings.
If you are a qualified Best Interest Assessor (BIA) and/or a warranted Approved Mental Health Professional (AMHP) we would love to hear from you. If not but you have some experience working in mental health and would be willing to train, we would encourage you to apply.
We are looking for candidates who:
- Have social work, occupational therapy, nursing or psychology qualifications and are registered with the professional regulatory body
- Have supervision skills demonstrated through the supervision of social workers or students including the assessment of competencies
- Can demonstrate confident and critical application of professional ethical principles to decision making and practice
- Are able to demonstrate accountability and effective judgement when anticipating, identifying, assessing and managing risk, seeking support and solutions where appropriate
- Have the ability to promote, develop and establish effective, collaborative partnerships and networks to promote appropriate information sharing, and utilise the experience of external agency professionals, service users and carers
- Can demonstrate knowledge of appropriate legal and policy frameworks, case law and the application to social work practice
