Senior Urgent Response Practitioner (Home-Based Treatment Team) – Wigan, Greater Manchester NHS
Are you a Qualified Mental Health Nurse, Social Worker, or Occupational Therapist with urgent care experience looking to progress and lead on the development of our Home-Based Treatment Team (HBTT)?
About the Role
Two Senior Practitioner opportunities are available within the Wigan Home-Based Treatment Team (HBTT). This is a senior clinical role focused on:
- Responding urgently to referrals from Inpatient, Mental Health Liaison (MHL) and Community Teams
- Providing alternatives to inpatient admission for individuals with complex presentations
- Overseeing patient care and service pressures, while proactively reducing them
- Supporting colleagues in HBTT to advocate for recovery at home in the least restrictive way
- Supervising and mentoring staff in professional development
- Leading service development projects within urgent care services
Key Responsibilities
The Senior Practitioner (Urgent Response) will be responsible for:
Clinical & Operational Leadership
- Provide senior clinical support across:
- Urgent Mental Health Liaison Services
- Home-Based Treatment Team (HBTT)
- Inpatient Services
- Community Teams
- Lead on High Intensity User Pathway (frequent attenders, recurrent crises)
- Support High Intensity Users—including those who frequently attend A&E, experience repeat inpatient admissions, or present in ongoing crises

Admission Avoidance & Service Coordination
- Provide senior clinical input into:
- Admission avoidance decisions
- Complex case presentations where inpatient admission is considered
- Support Supported Early Discharge efforts by:
- Assisting inpatient teams to identify early discharge opportunities
- Facilitating MDT liaison and in-reaching service users
- Ensuring ongoing care is provided safely by the HBTT
Risk Management & Complex Interventions
- Provide consultation and support in managing complex risk presentations, including:
- Repeated self-harm
- Emotional dysregulation
- Substance misuse
- Relational risk dynamics
Staff Support & Professional Development
- Provide line management supervision, advice, and reflective practice support for colleagues in:
- Urgent mental healthcare teams
- Contribute to the professional development of staff in the HBTT

- Participate in service development projects, including:
- Audit & service evaluations
- Quality improvement initiatives
- Developing urgent care pathways
