Senior Rehabilitation Case Manager - Exeter

Location: Exeter

Hours: 35-37.5 hours per week (part-time roles also considered)

Salary: £52,000 - £55,000/year + benefits

Job Overview

Continuing on our growth journey, Proclaim Care are now seeking experienced healthcare professionals with proven leadership and line management experience to join our expanding team.

Part of Proclaim Group, we are a national, award-winning provider of rehabilitation solutions for individuals who have been injured and/or are absent from work.

About The Role

This opportunity has arisen as part of our continued organisational evolution, including the development of our leadership structure and the progression of team members into new roles. As a result, we are looking to strengthen our leadership capacity across the business.

These roles are ideal for individuals who bring strong clinical or healthcare backgrounds alongside strong experience in leading people, managing performance, and supporting teams to deliver high-quality outcomes.

This is not limited to experienced case managers. We are keen to hear from professionals across a range of healthcare settings who have developed leadership capability and are looking to apply this within a dynamic, outcome-focused organisation.

This is a home-based role that involves significant regional travel. It is suitable for an applicant based in Salisbury or surrounding areas.

Summary Of Essential Requirements

Job Description

This is an exciting dual role that offers the opportunity to provide operational leadership and support to fellow team members, including delegated line management responsibilities as well as to support clients with complex injuries and health conditions to optimise their recovery and reach their rehabilitation and return to work goals.

Working within a commercial (personal injury and medical negligence) context, this field-based role involves navigating clients through their personal rehabilitation journey by:

  • Conducting in-person and virtual assessments to determine clients’ physical, psychological and social needs on an ongoing basis
  • Devising and coordinating goal-focussed rehabilitation programmes, which includes a combination of both private and statutory services
  • Coordinating private treatment e.g. physiotherapy, psychology, investigations or surgery
  • Collaborating with family members and other inter-disciplinary professionals
  • Communicating regularly with external stakeholders e.g. solicitors, insurers and employers
  • Writing comprehensive reports for the instructing parties to provide updates, demonstrate progress and seek funding approval
  • Facilitating return to work and/or education

The operational support and leadership component of the role involves:

  • Assisting the Regional Manager to ensure safe, effective, person-centred care aligned with company policies.
  • Conducting regular one-to-one meetings with Rehabilitation Managers to provide feedback, address challenges, agree priorities, and drive accountability.
  • Identifying training needs and promoting continuous professional development (CPD).
  • Coaching and mentoring Rehabilitation Managers on assessment, rehabilitation case management, and billing practices.
  • Supporting Rehabilitation Managers with developing effective strategies for meeting financial targets and SLAs.
  • Providing guidance on risk management and safeguarding responsibilities.
  • Facilitating team huddles and case clinics to enhance collaboration.
  • Offering ad hoc support for Clinical Duty rota and covering Regional Manager absence.
  • Managing annual leave and training approvals.
  • Supporting Regional Manager with recruitment, induction, and performance management.
  • Supporting the orientation, development and quality assurance of Associate Rehabilitation Managers, including audits and quarterly case reviews.

Person Specification

We are looking for a HCPC registered Occupational Therapist or Physiotherapist, or NMC registered Nurse with a proven clinical background in one or more of the following specialisms:

  • Complex orthopaedic conditions
  • Brain injury
  • Spinal cord injury
  • Amputations
  • Paediatrics

Our Senior Rehabilitation Case Managers work from home but regional travel to meet with clients, team members, treatment providers and employers is a large part of the role. They also meet frequently as a team (both virtually and in person) and attend regular training events. A valid driving licence is therefore essential.

  • Excellent organisational and administrative skills with attention to detail
  • The ability to work independently and as part of a multidisciplinary team
  • In-depth knowledge of rehabilitation case management, professional and regulatory standards and frameworks

  • Strong problem-solving skills with the desire to identify and resolve operational, people and customer issues.
  • Desire to inspire, develop and empower teams to foster a high-performance, collaborative culture.
  • Understanding of how own role contributes to positive outcomes, customer satisfaction and company profitability.
