Job Description
Salary: £40,777 - £46,142 per annum
Contract Type: Fixed Term until 31/12/2026
Hours: Full Time, 37 hours per week
Senior Social Worker - Hospital Discharge
This is an incredibly exciting time to join our service. We have launched Living Better Lives in Lancashire, our new strengths-based vision for Adult Social Care. Through this, we are embedding innovative practice that will make a real difference to the lives of people in our communities. We are looking for Senior Social Workers to support us in delivering our vision of supporting people to achieve what matters to them, living independently in a place they call home.
Collaboration and co-production are at the heart of what we do. Working alongside partners, community organisations, carers, and most importantly the people we support, to shape meaningful and personalised outcomes.
The Role
Joining our Hospital Discharge team as a Senior Social Worker, in this role you will support safe, timely, and person-centred discharges from hospital, working closely with health colleagues, families, and wider partners. You will help people move from hospital to the most appropriate setting, promoting recovery, independence, and a positive discharge experience.

Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
