Senior Social Worker
Senior Social Worker – Preparation for Adulthood Team
About the Role

Our Adults’ Social Workers deliver critical services across Initial Assessment, Locality and Hospital Teams, alongside our specialist Learning Disability Service. We are currently seeking a Senior Social Worker for our Preparation for Adulthood Team, focused on supporting young people transitioning into adult services.
Responsibilities
- Collaborate with:
- The Transition Service
- Children with Disabilities Team
- Leaving Care Team
- Special Educational Needs (SEN) Services
- Conduct timely and high-quality assessments, re-assessments, and reviews, providing personalised care to promote independence and safety
- Manage complex cases, including:
- Education, Health and Care Plans (EHCPs)
- Health Action Planning
- Person-centred planning
- Participate in Continuing Health Care (CHC) assessments where appropriate
- Support young people with employment transition planning
- Liaise with statutory organisations, voluntary bodies, and external agencies to ensure effective service delivery
- Represent the service in team and strategic developments, sharing insights and best practices
- Undertake:
- Mental Capacity Act assessments
- Best Interest determinations
- Community Deprivation of Liberty Reviews (DoLs)
- Manage safeguarding referrals, risk assessments, and protection plans
- Provide duty and senior support, offering practice guidance across Learning Disability Teams
About You
You must be a qualified and experienced Senior Social Worker eager to progress within Adults’ Preparation for Adulthood services.

Essential skills & experience:
- Proven competence in statutory duties (e.g., Care Act assessments, safeguarding processes)
- Ability to ensure smooth transitions for young people moving into adult social care
- Demonstrated creativity and innovation in developing new models of working to improve service outcomes
- Strong interpersonal skills—excellent at building trust-based relationships with vulnerable individuals
- Experience with person-centred planning, EHCPs, and health action planning
