Senior Transcare Nurse - Exeter
Clinical Homecare Nursing – Parenteral Nutrition & IV Therapies
Your Role
Our Clinical Homecare Nursing team is dedicated to delivering high-quality, individualised nursing care in partnership with NHS colleagues. We specialise in supporting patients requiring Parenteral Nutrition (PN) and other IV therapies in their own homes.

Key Details
- Location: Exeter and surrounding areas, working with local trusts and patients.
- Hours: 40 hours per week over 5 days (Monday to Sunday), with a rotating shift pattern.
- Shift structure:
- Typically 4 hours in the morning (disconnecting patients)
- 4 hours in the evening (reconnecting patients)
- Break during the day
- Flexibility is key—some shifts may vary in length based on patient visits.
- Shift structure:
- Salary: £43,391 per annum.
Main Responsibilities
- Administer prescribed IV therapies and provide tailored care for patients who cannot train to independence.
- Train, educate, and support patients/carers in:
- Equipment and infusion device use
- Connection/disconnection of PN and IV therapies (to independence)
- Care and management of Central Venous Access Devices (CVADs), aligned with local policies and national guidelines.
- Work as part of an on-call rota, making autonomous clinical decisions and escalating urgent issues to senior staff as necessary.
- Collaborate with the Practice Development Nurse and Regional Lead Nurse to ensure smooth discharge management, including:
- Pre-discharge assessments
- Home assessments
- Seamless co-ordination of care.
- Assist in TransCare patient installations by completing documentation and escalating any concerns.

Experience Required
- Equivalent experience to top-level Band 5 nursing, with specialised expertise in:
- Intensive Care / High Dependency / General Surgery Nursing
- Minimum two years of clinical experience in the last two years.
- Proven competence in:
- The management and delivery of PN and IV Therapies
- CVAD care and maintenance
- Blood sampling via CVADs.
What You Need to Succeed
- Registered General Nurse (NMC-registered).
- Preferably holds (or is willing to obtain within 12 months) a Mentoring/Teaching Certificate (or equivalent).
- Excellent communication and time-management skills with the ability to work under pressure.
- Strong clinical judgement and ability to make quick, accurate decisions in emergencies.
- High degree of flexibility—travel required to accommodate patients and carers.
