Senior Transcare Nurse
Clinical Homecare Nursing Role
Our Clinical Homecare Nursing team is passionate about delivering quality nursing care in partnership with NHS colleagues. We provide personalised, individualised care for patients requiring Parenteral Nutrition and other IV therapies in their own homes.

Key Details
- Location: South Wales and surrounding areas
- Working Hours: Monday to Sunday, averaging 40 hours per week over 5 days, on a rolling rota
- Shift Structure:
- Typically split shifts (~4 hours in the morning for disconnection, ~4 hours in the evening for reconnection)
- Oral schedules vary for patient visits but average to 40 hours weekly
- Flexibility essential
- Salary: £43,391 per annum
Main Responsibilities
- Administer prescribed IV therapies and associated care to patients who cannot train to independence
- Train, educate, and support patients/carers in:
- Equipment and infusion devices for therapy administration
- Connection and disconnection of Parenteral Nutrition (PN) and IV therapies to achieve independence
- CVAD (Central Venous Access Device) care and management in line with local policies and national guidelines
- Participate in an on-call rota, making autonomous clinical decisions and escalating to senior support if necessary
- Collaborate with the Practice Development Nurse and Regional Lead Nurse on discharge management, including:
- Pre-discharge assessment visits
- Home assessments and seamless service processes
- Support the insertion of new TransCare patients by:
- Completing all relevant documentation
- Escalating any issues promptly

Experience Required
- Clinical experience equivalent to top of Band 5 in:
- Intensive Care
- High dependency
- General surgery
- Minimum of 2 years clinical experience in the past
- Competent in:
- Management and delivery of Parenteral Nutrition (PN) and related IV therapies
- CVAD care and management
- Blood sampling from CVADs
Requirements
- Registered General Nurse with current NMC registration
- Either:
- Mentoring/Teaching Certificate, or
- Willingness to attain a certificate within 12 months
- Exceptional communication, time management, and adherence to strict timelines
- Ability to make sound clinical decisions in emergencies
- Flexibility to travel for homecare patient support as required
