Exciting Opportunity with South Manchester Home-Based Treatment Team
This is an exciting opportunity to take a lead role in contributing to the delivery of high-quality care within the South Manchester Home-Based Treatment Team. As a Band 7 Senior Practitioner, you will provide strong leadership and support the management of a busy urgent care team to ensure the delivery of safe, effective, and compassionate care for individuals requiring HBTT as an alternative to hospital admission.
Key Responsibilities
- Support the day-to-day coordination and management of the service.
- Provide visible and accessible leadership to staff within the Service.
- Support the delivery of a safe and efficient service to those accessing it.
- Ensure timely, person-centred interventions that promote recovery and wellbeing.
- Monitor the performance and quality of the service the team delivers.
- Provide clinical leadership and effective day-to-day management of the team.
Requirements
- Ability to manage your own workload.
- Ability to work well under pressure.
- Ability to work within a multi-disciplinary team.
- Excellent interpersonal skills.
- Awareness of Trust Policies and Procedures.
- Flexibility in working patterns, including weekend and out-of-hours working.
- Contribute to the on-call Operational rota.

Role Specifics
- Support the service to develop and maintain robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions.
- Proactively identify and facilitate supported early discharges from inpatient settings.
- Be involved in purposeful admission formulation and statements, and the review of these as part of ward rounds.
- Play an active part in the leadership and service development of the HBT.
- Facilitate spoke days for shadowing as part of induction or on student placements.
