Part Time Short Breaks Personalisation Officer
Contract Type: Permanent
Working Hours: 18.5 hours per week Worker Type: Hybrid Worker Salary: Starting at £32,597 (rising to £35,412 per annum pro rata for part-time), pending pay award Location: Hybrid working (team based at Loxley House, Nottingham)

About the Role
The Short Breaks Personalisation Officer supports families and disabled children/young people in accessing person-centred, high-quality short breaks services. Responsibilities include:
1. Enabling Access to Short Breaks Services
Core duties involve:
- Processing applications (from families or professionals) and de-escalating referrals from the Children’s Disability Service
- Screening applicants against eligibility criteria
- Assessing needs through:
- Completing application forms, reviewing supporting letters, and liaising with schools/other agencies
- Conducting home visits and holistic assessments (including capturing the child/young person’s voice)
- Utilising the Resource Allocation System (RAS) to determine support levels and budgets
- Conducting annual reviews for Pathway Two support packages and managing Pathway One re-applications
- Addressing enquiries with timely, sound responses

2. Supporting Personal Budgets
Help service users meet care/social needs through:
- Collaborating on person-centred support plans and mandatory documentation
- Identifying and sourcing paid/volunteer support services tailored to SEND (Special Educational Needs and Disability) outcomes
About You
We’re looking for a compassionate, skilled professional with:
Essential Requirements
- Deep knowledge of Personalisation and Person-Centred care models
- Understanding of the SEND Outcomes Framework
- Ability to develop and update individualised support/care plans (inclusive of home visits)
- Experience managing workloads independently, recognising when manager support is needed
- Strong administrative skills (maintaining electronic case files and systems)
- A DBS Enhanced Check (regulated activity)
