Job Opening: Part-time Short Breaks Personalisation Officer (Permanent position, Hybrid Work, 18.5 hrs/week)
Contract Details
- Type: Permanent
- Salary: £32,597–35,412 (pro rata for part-time) (pending pay award)
- Location: Hybrid (in office at Loxley House) + Remote
- Working Hours: 18.5 hrs/week

About the Role
The Part-time Short Breaks Personalisation Officer facilitates person-centred short breaks for disabled children/young people & families, ensuring equitable access to tailored support.
Key Responsibilities
Eligibility & Needs Assessment
- Screen applications (directly submitted by families/professionals or escalated from Children’s Disability Service)
- Assess eligibility for Pathway One & Two breaks via:
- Internal forms, professional letters, school/social network contacts, and the Resource Allocation System (RAS)
- Conduct home visits and reviews to gather insights
- Estimate financial budgets and determine service needs

Support Coordination
- Develop individualised support plans aligned with SEN outcomes
- Provide guidance on Personal Budgets, both formally and informally sourced
- Facilitate Social Care & Health consultations for care plans & direct payment applications
- Follow through on enquiries and ensure ongoing service relevance
About You

Essential Requirements

Equality & Human Rights knowledge, particularly regarding disabled children/young people

Personalisation/SEN Framework comprehension

Proven skills in:
- Writing care/Support Plans and related documentation
- Maintaining electronic case files
- Independent work, workload management, and time prioritisation

Demonstrated ability to raise concerns through the Nottingham City Council Code of Conduct
Eligibility Note
- Vital for the role to complete a DBS Enhanced Check
