Role Description

This role involves carrying out home visits, assessing the needs of older people, including those with mental health needs, people with physical disabilities, and their carers' needs in accordance with the Care Act 2014.

Key Requirements

  • Genuine interest in promoting independence and supporting people to regain their confidence and skills.
  • Develop and monitor reablement programs to empower people with activities that are important and meaningful to them.
  • Work within a multi-disciplinary team including care agency staff, occupational therapists, physiotherapists, and social workers to help people attain an agreed level of independence.

Desirable Experience

  • An understanding of reablement / rehabilitation and adult social care is desirable.
