We are seeking an enthusiastic and motivated Occupational Therapist to join a dynamic Community Reablement Team based in Dorset.
This rewarding role focuses on supporting adults following hospital discharge, enabling them to regain independence and safely remain within their own homes. Working as part of a multidisciplinary team, you will assess, plan, and deliver person-centred interventions to promote recovery, prevent readmission, and improve quality of life.
The role is predominantly community-based and will involve independent lone working across Bournemouth, Christchurch, and Poole.
Key Responsibilities
- Assess the functional, physical, and cognitive needs of adults following hospital discharge.
- Develop and implement individualized reablement and rehabilitation plans.
- Recommend equipment, adaptations, and support services to maximize independence.
- Work collaboratively with healthcare professionals, social care teams, patients, and carers.
- Manage a varied community caseload and prioritize workload effectively.
- Complete comprehensive assessments, documentation, and risk assessments.
- Promote safe discharge pathways and prevent unnecessary hospital readmissions.
- Participate in multidisciplinary meetings and contribute to service development.
- Adhere to HCPC standards, professional guidelines, and local policies.

Essential Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC Registration.
- Experience working within community, rehabilitation, reablement, or acute healthcare settings.
- Strong assessment, communication, and clinical reasoning skills.
- Ability to work independently and confidently in a lone-working environment.
- Full UK driving licence and access to a vehicle for work purposes.
- Excellent organizational and time management skills.

Desirable Experience
- Previous experience within Community Reablement services.
- Experience supporting adults transitioning from hospital to home.
- Knowledge of equipment provision, moving and handling, and home adaptations.
- Experience working within multidisciplinary teams.
