Warwickshire County Council

Warwickshire County Council is thrilled to announce an exciting position within our services! We are on the lookout for a passionate Social Care Worker to join our dynamic Reablement team. Our well-established team includes Social Care Practitioners, Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, and Reablement Assistants, all dedicated to achieving the best outcomes for our customers on their Reablement journey.

This is a fantastic opportunity to become a Social Care Practitioner in our county-wide Reablement Service. In this role, you will assess customers in their own homes, following a therapy-led care package provided by our team. We pride ourselves on a holistic approach that embraces multi-professional skills, empowering customer choice and control.

  • Key Responsibilities:
    • Work with customers aged 18 and over, addressing a range of functional difficulties, whether cognitive, physical, environmental, or psychological.
    • Base in either the North or South, with the flexibility to support the Reablement Service across the county during busy periods.
    • Assist the Hospital Teams when needed, with full support provided.
    • Support the Moving on Bed Service (MOB), which includes Residential settings and Extra Care Housing (ECH).

Work Schedule:

  • Monday to Friday
  • One Saturday every seven weeks
  • Opportunity to work on bank holidays on a rota basis
