Social Care Practitioner – Derbyshire Dales Mental Health Adult Care Team
About the Role
An opportunity has arisen within the Derbyshire Dales Mental Health Adult Care Team for a Social Care Practitioner.
The team provides support for individuals aged 18 and over with mental health difficulties.
Key Responsibilities
- Assessing care needs in line with the Care Act 2014 and Mental Capacity Act 2005
- Working collaboratively with clients and their support networks to develop personalised support plans
- Requires knowledge of the Mental Health Act and other relevant legislation
- Supporting independence using a person-centred, strengths-based approach
- Working within formal and informal support settings

Essential Requirements
- Strong understanding of safeguarding protocols
- Professional knowledge of mental health legislation and statutory frameworks
Working Arrangement
- Hybrid working model (currently home or office-based, flexible based on service needs)
