Social Care Practitioner (Penwith East)

Social Care Practitioner (Penwith East PCN Team) – Full-Time Position

About the Service & Team

We are recruiting for one full-time Social Care Practitioner within the Penwith East Primary Care Network (PCN) team, covering St Ives, Carbis Bay, and Hayle areas.


The Role

Under the leadership of the Team Manager, you will work closely with Social Workers and other Social Care Practitioners to deliver exceptional care to adults with health and social care needs.

Your responsibilities will include:

  • Conducting timely, high-quality, and person-centred needs assessments
  • Developing and implementing support plans to enhance independence and improve outcomes for vulnerable adults
  • Collaborating with colleagues across different disciplines and partner organisations to ensure a seamless service
  • Protecting vulnerable adults from harm/abuse, while supporting them to live as independently as possible
  • Working with communities, individuals, and families to strengthen personal strengths

Key Requirements of the Role:

  • The public-facing nature of this position means adherence to English language requirements for public sector workers applies
  • Enhanced criminal record disclosure check will be conducted

Working Pattern: Monday to Friday: 8:45 AM – 5:15 PM

What You’ll Need to Succeed

To excel in this role, you should possess:

  • At least a Level 3 qualification in health, social care, or a related field—or a commitment to working towards appropriate qualifications.
  • Strong interviewing and assessment skills, including the ability to evaluate risks, strengths, and support needs.
  • Exceptional analytical abilities to translate complex information into clear, actionable plans and outcomes.
  • Creativity and resilience in high-pressure situations—with a willingness to seek support when needed and escalate to more experienced colleagues.
  • Flexibility and prioritisation skills to handle urgent referrals effectively.
