We are looking for a passionate, person-centred Social Care Practitioner to join our Erewash Short Term Assessment and Reablement Team (START). This is a permanent, full-time opportunity based in Erewash, offering a varied and rewarding caseload.
About The Team
Erewash START provides a multi-disciplinary approach to assess, support, and enable adults to maximise their independence and identify any potential long-term care needs. We are committed to working in a person-centred and strengths-based way, recognising people’s assets and enabling them to reach their goals. While community-based, the team also supports individuals transitioning from residential and nursing establishments and facilitates safe and timely hospital discharges. We offer a hybrid working model, with staff working from home and the office depending on service needs.
About The Role
This is a role for someone who enjoys pace and variety — but who also wants their work to have a clear purpose. We want to hear from someone who can:
- Deliver strengths-based, holistic assessments to support adults, carers, and families.
- Support the review of goals and progress against these using feedback from the MDT.
- Promote independence, choice, and wellbeing through creative problem-solving and personalised interventions.
- Work across a variety of areas including health and social care needs.
- Be part of a duty system that undertakes initial safeguarding enquiries and triage for all new referrals.
- Assess for and provide small aids and tech as part of a full assessment.
- Complete assessments and support plans in line with the Care Act 2014, to enable people to remain as independent as they can safely be.
- Collaborate with a multidisciplinary team and contribute to inter-agency working across health and social care.
- Refer and signpost to other services as appropriate.

What You’ll Be Doing
As a Social Care Practitioner in START, you’ll:
- Complete strengths-based assessments and reviews for adults with less complex needs, focusing on what matters to the person and what they can achieve with the right support.
- Arrange support to meet eligible needs, with a clear focus on enabling progress and reducing reliance on ongoing care where appropriate.
- Work on a duty rota, triaging new referrals and responding to urgent situations with calm judgement and clear prioritisation.
- Build positive working relationships with health colleagues and partner agencies to support safe decision-making and timely plans.
- Contribute to a team culture where we review, learn and improve — and where good recording and good outcomes matter.

Who We’re Looking For
We’re looking for someone who:

- Brings a warm, practical, respectful approach to working with people and families.
- Can work at pace, prioritise effectively and make proportionate decisions.
- Communicates well and enjoys working as part of an MDT.
- Takes pride in safe, accountable, efficient practice — and wants to keep developing.
- Has a good understanding of the Care Act 2014.
