Social Prescriber
Social Prescriber – York Community Wellbeing Hubs
About the Role
As part of the Yor Community Wellbeing Hubs, Social Prescribers are integrated into a broader network of delivery across voluntary sector, local authority, and health services.
The post holder will play a vital role in supporting individuals to develop personal independence and resilience, enabling self-management of health and wellbeing. This will be achieved through individualised co-produced action plans, tailored to specific needs. The role requires:
- Strong relationship-building to establish trust quickly with individuals who may have serious mental health conditions
- Motivational support to encourage sustained behavioural change
- Sensitive and community-focused intervention to overcome barriers to accessing support
Social Prescribers will also link individuals with local resources by collaborating with services and activities that address social and health needs, effectively signposting where appropriate and enabling participation in community-based initiatives.
Key Responsibilities
- Supporting vulnerable individuals — Work with people accessing Wellbeing Hubs and those transitioning from Foss Park Hospital under the Pathway to Recovery (P2R) Project.
- Building resilience and independence — Empower individuals to manage their health and wellbeing through personalised intervention.
- Facilitating access to activity — Help individuals access community-based programmes designed to improve physical health and mental resilience within agreed time frames.
- Assess and monitor progress — Use recognised tools to track outcomes and measure impact.
- Multi-agency collaboration — Operate within a matrix management structure, ensuring coordination with voluntary, statutory, and public sector partners.
- Promoting health equity — Strengthen multi-agency working across communities to address and reduce health inequalities.

- Strong therapeutic and motivational skills — Ability to connect quickly with mentally vulnerable individuals.
- Mental health awareness — Experience (or empathy) in supporting people with serious mental illness.
- Cross-sector partnership experience — Experience working with local authorities, health providers, and voluntary organisations.
- Strength-based engagement — Ability to overcome barriers and enable long-term change.
- Excellent record-keeping — Use structured assessment tools to document progress.
- Community-focused approach — Commitment to embedding social and health interventions in local settings.
