Project Support
The purpose of the Social Prescribing Service is to put older people in personal control of their health & wellbeing and provide them with integrated care and support in their local community, focusing on prevention of ill health and the promotion of wellbeing.

The Social Prescribing Service aims to enable older people to have improved health outcomes and independence whilst reducing the burden on NHS services in Sunderland through the reduction in need for GP visits, out of hours contacts, and emergency admissions/readmissions.
The successful applicant will be responsible for the delivery of the Social Prescribing Service Programme across the City of Sunderland. A good standard of education is required at NVQ level 3 level preferably in Health & Social Care.
