Job Details

We are looking for a dynamic, enthusiastic and motivated Social Work Assistant to join the Social Work Hospital Discharge Team (West). The team works with adults who have physical and/or mental health needs, their families and carers promoting a person-centred approach to discharge planning. The West team covers Victoria Hospital and Queen Margaret Hospital as well as Fife residents in hospitals such as Lothian and Falkirk.

The successful candidate will work collaboratively with social workers contributing to personal outcome focused assessments as part of discharge planning. They will engage with individuals, families and carers to address some of the practical barriers to discharge whilst promoting their rights and strengths to achieve better outcomes.

  • 36hrs per week Monday to Friday on a permanent basis with a salary of £31,761.26 - £34,914.86
  • Flexible working with core hours 8am – 4pm.

The person

The post-holder will:

  • Possess active listening skill, demonstrate empathy, clarity and be able to understand and be understood.
  • Have awareness of the assessment process including assessment of risk and risk management.
  • Possess good report writing skills.
  • Be able to work effectively with a range of professionals.
  • Have the ability to remain calm and professional under pressure, be able to work independently as well as part of a team.
  • Have experience of working in a relevant setting. (multidisciplinary working is desirable).
  • Have an awareness of personal centred practice and practice in a way which empowers individuals.
  • Ability to think creatively and have excellent problem-solving skills.
  • Have an ability to establish and maintain a professional relationship with service users.
  • Have the ability to travel throughout Fife.
  • Have good IT skills.
