Are you passionate about making a difference to our citizens and the district that they reside in? Then, we have an exciting opportunity for you to join our Locality Social Work Team based at Waterton House, Wakefield.

About The Role

We are seeking an individual who is passionate about adult social care and providing a high-quality service and is keen to develop their professional experience and knowledge further. You will play a key part in delivering our adult social care strategy and will be committed to strengths-based and outcome-focused practice. As a member of the team you will ensure that the service delivers positive outcomes for people. You will have a strong commitment to human rights; you will ensure the person’s voice is heard and acted upon.

Key Responsibilities

  • Managing your own caseload.
  • Completing Mental Capacity Assessments and Best Interest Decision making.
  • Part of a duty rota within core hours.
  • Working with a range of teams across the Council and our partners including primary and secondary health services, voluntary and private sector services.
  • Undertaking Care Act assessments, Carers Assessments, co-producing care and support plans and reviewing people’s identified outcomes.

What will be expected from me?

  • Strong practice knowledge – an understanding of adult social care, the Care Act 2014 and the Mental Capacity Act 2005, and the confidence to apply them.

Essential Requirements

Applicants for this role must be qualified to a Degree level or equivalent and be registered as a Social Worker with Social Work England registration. The successful person must hold a valid and current driver’s license and have the use of a car.
