Social Worker
Social Worker – Learning Disability & Autism Programme (Part-Time, Hybrid)
Salary £39,152 - £44,075 per annum (pro rata)
Contract Type Permanent
Working Pattern Part-time (18.5 hours per week)
Location Welbeck House, Sherwood Energy Village, NG22 9FF
Working Arrangements Hybrid (office and home-based)
About the Role
This is an exciting opportunity to join the Transforming Lives Team. As with this role, you will provide specialist support to adults with learning disabilities and autism, ensuring their needs are met under the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
Your responsibilities will include:
- Conducting Care Act assessments
- Developing support plans
- Assessing and managing risks
- Commissioning and coordinating discharges
- Working collaboratively with external agencies and providers
This is a dynamic role requiring:
- A creative approach to supporting adults with complex care needs
- An unwavering commitment to strengths-based practice, promoting independence and wellbeing
- Strong risk management, ensuring safety while fostering autonomy
The team works autonomously and in partnership, so high levels of confidence, resilience, and interprofessional collaboration are essential.

Key Responsibilities
- Meet regulatory and organisational requirements including Care Act, Mental Capacity Act, and Mental Health Act compliance.
- Deliver Care Act assessments and develop individualised support plans.
- Assess and manage risks for individuals under your care.
- Work with external agencies, commissioner services, and providers to streamline discharges.
- Foster independent living through strengths-based approaches.
- Maintain high communication, negotiation, organisational, and IT skills.
- Adhere to hybrid working policies (partially office-based, partially remote).
Essential Requirements
You must:
- Be registered with Social Work England.
- Be an experienced Social Worker (proven track record in working with adults with care needs).
- Demonstrate exceptional communication, negotiation, organisation, and IT proficiency.
- Have a proven commitment to interprofessional collaboration.
- Work autonomously with a high degree of resilience.
- Hold a full valid UK driving licence and have access to a vehicle (taxi support provided if required due to disability).
