Social Worker

Part-Time Social Worker - Transforming Lives Team

Role Overview

Salary: £39,152 - £44,075 per annum (pro rata) Type: Permanent Work Pattern: Part-Time (18.5 hours per week) Work Base: Welbeck House, Sherwood Energy Village, NG22 9FF Working Arrangements: Hybrid


About the Role

This is an exciting opportunity to join the Transforming Lives Team at Nottinghamshire County Council. As a valued member of this team, you will specialise in supporting adults with significant care and support needs under the *Learning Disability and Autism Programme.

Key responsibilities include:

  • Conducting Care Act 2014 assessments
  • Developing support planning
  • Undertaking risk assessment tasks
  • Liaising with external agencies and commissioning services
  • Implementing planned discharges for service users

The role requires:

  • A passionate commitment to strength-based practice, promoting independence and wellbeing while managing risks effectively.
  • Experience of working creatively with individuals presenting complex care needs.
  • Collaboration with partner agencies in a fast-paced environment that may require swift decision-making and resilience.

The team operates a hybrid model, allowing for 18.5 hours per week, split between office-based and home working.


Key Responsibilities

  • Apply the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983 consistently in assisting service users.
  • Lead Care Act assessments to determine care and support needs.
  • Collaborate with providers and external agencies to plan and deliver personalised support.
  • Risk assess individuals to ensure safety and maximise independence where possible.
  • Liaise with relevant third parties (including families, healthcare workers, and local authorities) to coordinate seamless care pathways.
  • Ensure safe discharges for appropriate service users, involving discharge planning and coordination with community services.
  • Participate in interprofessional discussions, effectively communicating with stakeholders across sectors.
  • Maintain detailed documentation and operate IT and organisational tools efficiently.
  • Embrace a proactive, strengths-focused approach, consistently promoting advocacy for service users.

Requirements

Essential

  • Registered with Social Work England.
  • Experienced Social Worker in a similar clinical setting.
  • Effective communication, negotiation, organisational and IT skills.
  • Committed to interprofessional collaboration with partner agencies.
  • Ability to work autonomously with a strong degree of resilience.
  • Valid UK driving licence (or eligibility to use a taxi service provided by the Council).

Desirable

  • Experience working within the Learning Disabilities/Autism sector.
  • Familiarity with the Nelson-Thomson Assessment or similar tools.
  • Prior engagement with discharge planning and inter-agency processes.

Working Environment & Support

Hybrid & Flexible Working Model

You will be divided between:

  • Office-based work at Welbeck House, Nottingham.
  • Remote working from home (specific arrangements will be discussed during recruitment).
