Social Worker

Permanent Part-Time Social Worker (Band B) – Transforming Lives Team

Salary

£39,152 – £44,075 per annum (pro rata)

Work Details

  • Hours per week: 18.5
  • Work Base: Welbeck House, Sherwood Energy Village, NG22 9FF
  • Working Arrangement: Hybrid (part office, part remote)

About the Role

This is an exciting opportunity to join the Transforming Lives Team, specialising in supporting adults with social care needs under the Learning Disability and Autism Programme.

As a valued member of this team, you will:

  • Conduct Care Act 2014 assessments
  • Provide support planning & risk assessments
  • Commission and facilitate discharges for individuals with complex care requirements
  • Work collaboratively with external agencies and care providers

This role requires a strong understanding of: ✔ Care Act 2014 ✔ Mental Capacity Act 2005 ✔ Mental Health Act 1983


Responsibilities

You will be expected to:

  • Work autonomously and creatively with adults needing care and support.
  • Apply strengths-based practice, promoting independence and wellbeing whilst ensuring safer risk management.
  • Engage with a diverse range of stakeholders, including intermediaries, commissioners, and service providers.
  • Maintain flexible working practices, leveraging strong negotiation, organisational, and IT skills.
  • Assess and commission support packages tailored to individual needs.
  • Provide ongoing formal and informal supervision to support effective case management.
  • Work within hybrid models, balancing office-based and remote tasks.

Requirements

Must Have:

✅ Registered with Social Work England ✅ Experienced Social Worker with:

  • Knowledge of adult social care legislation
  • Skills in assessment, support planning & risk management
  • Commitment to strengths-based and safeguarding principles ✅ ability to work autonomously with resilience in a fast-paced environment. ✅ Effective communication, negotiation, organising & IT skills.

Essential Needed for the Post:

📋 Ability to work within a collaborative, often complex care landscape. 📋 Comfortable driving & having use of a vehicle for work (taxi provided if required).
