Social Worker – Transforming Lives Team (Part-Time, Hybrid)
About the Role
Join Nottinghamshire County Council in an exciting opportunity within the Transforming Lives Team. This role offers a meaningful chance to support adults with complex care and learning disabilities needs under the Learning Disability and Autism Programme.
You will work on Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983 assessments, discharge planning, risk management, and commissioned support plans. This career-focused role requires proactive collaboration with external agencies, providers, and multidisciplinary teams.
The team operates in a hybrid model (part office, part remote work) and provides careful supervision, supporting professional growth through regular training and mentoring.
Responsibilities
- Conduct Care Act 2014 assessments to identify care and support needs
- Develop and implement support plans, ensuring strengths-based and independence-focused approaches
- Commission and manage risk assessments and discharge plans
- Work collaboratively with external agencies (healthcare, care providers) to ensure effective co-ordination
- Promote personalisation, wellbeing, and safety in complex cases
- Drive flexible working practices utilising strong communication, negotiation, and organisational skills
- Operate a flexible driving policy (with dedicated taxi arrangements for disability access)

Requirements
You Must Be
- Registered with Social Work England
- An experienced Social Worker with specialist knowledge of:
- Learning Disabilities
- Autism Spectrum Disorder (ASD)
- Mental Capacity Act 2005 & Mental Health Act 1983
- Skilled in:
- Effective communication (including negotiating with diverse stakeholders)
- Organisational management (using IT systems for case management)
- Committed to interprofessional collaboration, flexible working, and autonomous casework
- Able to drive (car provided) or have alternative mobility arrangements (taxi support available)

Additional Correctives
- Must undergo a Disclosure and Barring Service (DBS) check

Salary & Contract Terms
- £39,152 – £44,075 per annum (pro-rated for 18.5 hours per week)
- Temporary, Hybrid Work Pattern (Welbeck House, Sherwood Energy Village, NG22 9FF)
