Job Details
We are looking for a dynamic, enthusiastic and motivated Social Worker to join the Social Work Hospital Discharge Team (West). The team works with adults who have physical and/or mental health needs, their families and carers promoting a person-centred approach to discharge planning. The West team covers Victoria Hospital and Queen Margaret Hospital as well as Fife residents in hospitals such as Lothian and Falkirk.
You will work as part of a multidisciplinary team undertaking personal outcome focused assessments to support adults to achieve safe and timely discharges.

36hrs per week Monday to Friday on a permanent basis with a salary of £43,437.09 to £47,604.34
Flexible working
The Person
The post-holder will:
- Undertake person outcome focused assessments for adults who are in hospital to achieve timely discharges in line with legislation and policy
- Complete comprehensive risk assessments and risk management plans
- Work collaboratively with a range of professionals
- Have experience in adult social work or a health-related setting. Experience of hospital social work is desirable
- Be able to manage complex cases and work under pressure
- Possess good report writing skills
- Have the ability to travel throughout Fife.
- Have good IT skills
Blended Working
