Transitions Social Worker

We are looking to appoint a Transitions Social Worker to join our newly developed Preparation for Adulthood (PfA) Service within SEND Central in York.

This role sits within Adults Social Care and works at the interface between children’s and adult services, supporting young people with SEND to move into adulthood in a timely, planned and person-centred way, in line with York’s Preparation for Adulthood vision.

You will be part of a multi-agency team, working closely with Children’s Social Care, SEND, education, health and wider partners to deliver a coordinated and consistent approach to transition planning, starting early and focused on strength-based outcomes.

You will play a key role in a multi-agency approach to Care Act assessments and transition planning, ensuring timely support, promoting independence and reducing delays or a “cliff edge” into adult services.

You Will

  • Lead on Care Act assessments and support planning
  • Manage a complex caseload
  • Work collaboratively across agencies to support effective, outcome-focused transitions
  • Contribute to early planning (from Year 9 onwards)
  • Promote strengths-based practice, supporting independence, employment, community inclusion and positive adult lives

About You

  • Qualified Social Worker, registered with Social Work England
  • Experience of complex practice and multi-agency working
  • Strong knowledge of the Care Act and Mental Capacity Act
  • Confident in decision-making and managing complexity
  • Committed to supporting young people to achieve independence and positive outcomes

We are not currently licensed to sponsor employees for visas, therefore are unable to offer sponsorship for this role.

This role is suitable as a job share opportunity.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1: Can express themselves fluently and spontaneously, almost effortlessly. Only a conceptually difficult subject can hinder a natural, smooth flow of language.

Hybrid working arrangements are available for this role. Each role at CYC is different and your working arrangements will be determined based on the requirements of the role and in collaboration with you and your needs as part of the recruitment process. Your contractual location will be the designated council office for your team.
