About The Team
The Adult Social Care Hospital Team collaborates with local community hospitals to deliver:
- Short-term assessments
- Support for the Discharge to Assess (D2A) pathway
- Safeguarding responses
We’re expanding the team: 3 Social Worker vacancies are available in a supportive and collaborative environment. You’ll work alongside:
- Service Manager
- Team Manager
- Social Workers
- Social Care Practitioners
- Administrative staff
You’ll partner closely with the Community Pathways Team to:
- Assist individuals transitioning home with reablement
- Ensure ongoing recovery, review, and safeguarding.
About The Role
This role centers on delivering high-quality, strengths-based social work for:
- Adults
- Carers
Key focus areas:
- Facilitating safe and timely discharges from hospital using the Discharge to Assess (D2A) model.
- Completing the initial community review after discharge.

Responsibilities include:
- Assessing needs in the most appropriate settings (hospitals, residential placements, or at home).
- Prioritizing independence while managing risk effectively.
Safeguarding is a core responsibility—protecting, supporting, and empowering individuals through a multi-agency approach.
Key Responsibilities
- Undertake strengths-based Care Act assessments for adults and carers.
- Collaborate on Discharge to Assess (D2A) processes (timely discharges + assessments).
- Lead and participate in safeguarding enquiries (Care Act s.42), including Making Safeguarding Personal.
- Develop and review person-centred support plans (promoting independence/wellbeing).
- Conduct Mental Capacity Assessments and Best Interest decisions.
- Manage an allocated caseload (prioritizing progress through the system).
- Maintain comprehensive case recordings, safeguarding documentation, and reports.
- Work collaboratively with health teams, families, housing providers, and voluntary partners.
- Advocate for early intervention, prevention, and community resources access.
