Social Worker - All Age Disability Team
All Age Disability Team Social Worker – Wrexham (Initial 3-Month Contract)
Liquid Personnel is recruiting for an experienced Social Worker to join an All Age Disability Team in Wrexham. This role focuses on delivering high-quality support to children, young people, and adults with lifelong disabilities through a holistic, strengths-based approach.
Key Responsibilities
Holistic Disability Assessment
- Complete detailed strengths-based assessments and reviews for individuals with lifelong disabilities
- Ensure compliance with Care Act (2014) and Children Act (1989) duties
- Foster individualised support plans that adapt across the lifespan
Safeguarding & Complex Needs
- Manage safeguarding concerns inclusively
- Conduct Section 42 enquiries where required
- Apply the Mental Capacity Act (2005) appropriately
- Lead and coordinate risk management plans
Integrated Support Planning
- Develop outcome-focused, person-centred support plans
- Collaborate with multi-agency teams (health, education, SEND providers, therapy services, and community support)
- Ensure seamless transitions during pivotal life stages (e.g., school–work, care home placement, adulthood)

Requirements & Qualifications
To apply, you must meet the following criteria:
- Registration with Social Work England
- Eligibility to work in the UK
- Possession of a full UK driving licence
- Post-qualified experience as a Social Worker
- A degree-level qualification in Social Work or an equivalent level of proficiency
