Liquid Personnel is recruiting an experienced Social Worker – Community Review for its client’s Adult Community Review Team. This is a rewarding opportunity to work within a supportive and friendly team, delivering high-quality assessments and reviews for adults with a range of care and support needs while benefiting from hybrid working, regular supervision, and ongoing training.
What will your responsibilities be?
As a Social Worker within the Community Review Team, you will:
- Complete Care Act assessments and reviews for adults with a range of physical, sensory, and social care needs.
- Identify eligible needs and develop person-centred support plans that promote independence, wellbeing, and positive outcomes.
- Undertake Mental Capacity Act (MCA) assessments to support decision-making and ensure individuals' rights are upheld.
- Carry out risk assessments and safeguarding enquiries, ensuring statutory responsibilities are met while promoting choice, control, and least restrictive practice.
- Manage a countywide caseload, prioritising work effectively and maintaining accurate case records.
- Work collaboratively with service users, families, healthcare professionals, and commissioned providers to coordinate care packages and support planning.
- Monitor and review care arrangements, ensuring services remain appropriate and responsive to changing needs.
- Facilitate access to community resources, support networks, and services that help individuals achieve their goals and maintain independence.

Qualifications and Experience Required
- Qualified Social Worker with a recognised Social Work qualification
- Social Work England registration
- Experience completing Care Act assessments and reviews
- Knowledge of the Mental Capacity Act, safeguarding legislation, and adult social care practice
- Ability to manage a complex caseload and work autonomously
- Strong assessment, safeguarding, and communication skills
