- Starting salary: PO2 – PO4 (£42,771 – £52,194)
- Work location: 48 Station Road, Wood Green, London, N22 7TY
- Hours per week: 36 per week
- Contract type: Permanent

About The Role
We are seeking experienced and motivated Specialist Continuing Healthcare (CHC) Social Workers to join our developing and dynamic CHC service. This is an exciting opportunity to be part of a newly established specialist team, playing a key role in delivering high-quality Continuing Healthcare assessments and decision-making.
You will work closely with ICB Nurse Assessors to complete Decision Support Tools (DSTs), alongside undertaking:
- Care Act assessments
- Reviews and support planning
- Adults with complex and rapidly changing needs

The role requires strong partnership working across multi-disciplinary teams, ensuring that individuals receive appropriate, timely, and person-centred care packages, with clear alignment between health and social care responsibilities.
Your responsibilities include:
- Managing a complex caseload
- Robust analysis of needs across CHC domains
- Contributing to eligibility decision-making
- Ensuring care packages are reviewed and remain proportionate and effective
A key part is to ensure CHC processes are delivered in line with the National Framework for NHS Continuing Healthcare, supporting lawful, consistent, and defensible decision-making while maintaining a strong focus on outcomes for residents.
About The Team
Haringey’s Continuing Health Care Team is a newly established, specialist service within Adult Social Care, created to strengthen and embed high-quality CHC practice across the borough. The team consists of skilled practitioners who work at the interface of health and social care, supporting residents with complex, long-term health needs.
We collaboratively work with NHS partners, providers, and internal services to deliver consistent, transparent, and person-centred CHC processes.
About You
We are looking for a proactive and knowledgeable practitioner who can bring both expertise and a collaborative approach to this specialist role.
Key Requirements
- Current registration as a Social Worker with Social Work England (SWE)
- Significant experience working with adults who have complex, long-term, or unpredictable health and care needs
- In-depth knowledge of:
- Care Act 2014
- Mental Capacity Act
- Safeguarding frameworks
- National CHC Framework
- Proven experience in:
- Completing or contributing to CHC assessments and DSTs
- Analysing evidence across CHC domains
- Strong skills in:
- Strengths-based assessments
- Care planning
- Case management
- Confidence in working within multi-disciplinary settings, influencing decision-making, and advocating for service users
- Exemplary analytical and recording skills, to produce clear, evidence-based reports
- Ability to:
- Balance competing priorities
- Work autonomously
- Maintain high professional standards
- Commit to person-centred practice, equity of access, and continuous service improvement
