Social Worker
Social Worker – Adult Social Care (Full-time) | Eastbourne & Conquest Hospital Community Team

About the Role
We are seeking a motivated Social Worker (SCPW/Class SC) to join the Eastbourne & Conquest Hospital Community Team, based at St Mary’s House, Eastbourne.
As a Social Worker in Adult Social Care, your primary focus will be:
- Assessing adults recently discharged from hospital to determine their ongoing care needs.
- Supporting care planning to maximise independence and improve outcomes.
- Implementing statutory frameworks in daily practice, including:
- Care Act (2014)
- Mental Capacity Act (2005)
- Equality and anti-discrimination legislation
- Deprivation of Liberty Safeguards (DoLS regulations)
- Carrying out safeguarding duties, protecting and mitigating risks for vulnerable adults.
You will work closely with hospital trusts, local partners, and integrated services, including Integrated Commissioning Boards (ICBs), to ensure seamless access to continuing care funding where applicable.
Who We’re Looking For
We are seeking registered Social Workers who demonstrate:
- Registration with Social Work England.
- Excellent communication skills, able to work effectively with diverse individuals, families, and professionals.
- A deep commitment to helping others achieve positive outcomes and a proactive attitude.
