Older People’s Mental Health Team (OPMHT) – Social Worker
Job Overview
- Hours: 37 hours per week
- Contract Duration: Temporary, 12 months from September 2026
- Location: Quarella Road, Bridgend
The Older People’s Mental Health Team (OPMHT) provides essential support to individuals aged 65+ in receipt of secondary mental health services, as defined by the Mental Health (Wales) Measure 2010. This multi-disciplinary team collaborates closely with:
- Health colleagues (Community Mental Health Nurses, Occupational Therapists, Physiotherapists)
- Angelton Clinic (often within Glanrhyd Hospital), supporting inpatient care for older adults with serious mental health conditions and dementia
- Consultant Psychiatrists and their teams
Key Responsibilities
Core Role
As a Social Worker within this strengths-based and outcome-focused team, your role includes:
- Providing person-centred care for individuals with complex, long-term mental health, physical health, and emotional well-being challenges.
- Working collaboratively with the individual, their families, and other disciplines to undertake holistic assessments.
- Developing and implementing individualised Care and Treatment Plans, ensuring outcomes are met.

As a Care Coordinator, you will:
- Jointly coordinate care and treatment with individuals to support their recovery and wellbeing.
- Undertake risk assessments and ensure adherence to welfare rights policies and safeguarding guidelines.
- Promote Independence, autonomy, and self-management, ensuring choices are respected.
- Focus on prevention and sustainable independence through practical interventions. -(uphold choice and control)—putting the person at the centre of decision-making.
Requirements
Essential Criteria
✔ Qualified Social Worker with a valid SWRB registration (or equivalent). ✔ Experience working with older adults (65+) with mentally ill populations. ✔ Strong skills in outcome-focused assessment and collaborative planning. ✔ Familiarity with the Mental Health (Wales) Measure 2010 and safeguarding duties. ✔ Must hold a valid UK driving licence (due to role requirements). ✔ Fluency in Welsh for direct interaction with service users (GREC requirement). ✔ Commitment to protecting vulnerable adults and children.
Desirable
- Experience working in multi-agency settings, including health and social care.
- Knowledge of dedicated mental health assessment and treatment pathways for older age groups.
