Adult Social Care Assessor – Portsmouth Adults Intermediate Care Team
Full-Time | Permanent | Portsmouth | £39,151 - £42,838 per annum | Closes: 12 July 2026
About the Role
You will join the Portsmouth Adults Intermediate Care Team as a Social Worker or Social Care Associate Practitioner. Your role involves:
- Assessing the needs of adults and their carers to develop creative, person-centred support plans comply with the Care Act 2014.
- Delivering outcome-led interventions using a strength-based approach to empower individuals to live the lives they choose.
- Collaborating with colleagues to apply an aspirational, strength-based philosophy to achieve meaningful, tailored outcomes.
- Working independently while managing your own caseload.

Core Responsibilities
- Conduct needs assessments and agree personalised support plans in line with legal and best practice frameworks.
- Complete assessment reviews and support plan evaluations.
- Provide holistic support addressing an individual’s health, mental health, and social care needs.
- Assess mental capacity where required.
- Supervise and mentor unqualified junior staff.
- Demonstrate professional curiosity and drive your own personal and professional growth.
Person Specification
Essential Criteria
- Hold a qualification in Social Work and be registered with Social Work England.
- Demonstrate expertise in safeguarding adults and a comprehensive knowledge of adult social care legislation.
- Proven ability to communicate effectively with a diverse population, including in challenging situations.
- Strong risk management skills with confidentiality and clear record-keeping.
- Ability to work collaboratively while championing the rights of individuals and carers.
- Experience supervising and supporting colleagues.
