Specialist Nurse for Care Homes – Greenwich
About the Role

An exciting opportunity has arisen in Adult Community Physical Health Services for a Specialist Nurse for Care Homes in Greenwich. Join us as we develop transformation projects like our Virtual Ward programme, keeping patients out of hospital and supporting community-based care.
As part of the Complex Case Management in Care Homes service, this role operates under the Enhanced Health in Care Homes (EHCH) framework. You’ll act as a Senior Nurse, collaborating with Primary Care, community teams (Care Home Managers, Social Care), social work, and multi-disciplinary teams (MDT) to optimise resident care, reduce hospital admissions, and maintain high standards in care homes.
Key responsibilities include coordinating EHCH implementation across Greenwich, managing interventions with input from:
- Care Home Staff
- Geriatricians
- Lonly Advisory & Support (LAS)
- GPs & Primary Care professionals
- Adult Social Care
- Other specialists where needed

Key Responsibilities
- Lead multi-disciplinary initiatives to support residents with complex chronic conditions, focusing on older adults with frailty.
- Work with MDTs to develop interventions that improve health and wellbeing while keeping residents safe at home.
- Drive Oxford model best practices and align with Greenwich’s Home First Virtual Ward goals.
- Foster high-functioning relationships among diverse stakeholders (clinicians, social workers, care staff).
- Focus on early discharge management, preventive strategies, and reducing inappropriate hospitalisations.
- Collaborate across sectors, including NHS, local authorities, and voluntary partners.
Required Skills & Qualifications
- Direct experience in nursing for older adults (preferably within care homes).
- Proven capability in multi-agency working—leading diverse teams and collaborating with medical colleagues.
- Strong skills in complex case management (frailty, dementia, or long-term illness).
- Commitment to quality improvement and patient-centred care.
- Experience coordinating Virtual Ward or community resilience models (advantageous).
- Living or working in east/lower London for effective integration with stakeholders.
