Location: Barnsley, South Yorkshire Contract: Permanent, Part-time (28.5 hours), Flexible working Salary: £39,959 to £48,117 per year (pro rata) Band: Band 6 (Agenda for Change)
Job Summary
An exciting opportunity for a Specialist Nurse with South West Yorkshire Partnership NHS Trust. This role involves supporting UK new entrants, marginalised clients, and socially excluded individuals—including asylum seekers, homeless residents, and gypsy/traveller communities—to improve their health and well-being.
The ideal candidate will:

About the Role
This clinic-based and street Outreach role focuses on vulnerable groups facing elimination of health disparities due to multiple social and health risks. Throughout the borough of Barnsley, the Health Inclusion Team partners with:
- Barnsley Council (Housing & Substance Misuse Services)
- Street-based outreach
- Wound care clinics
- Gypsy/Traveller sites
You will:
- Refer individuals to NHS, voluntary, and social support services
- Provide care coordination and GP registration support
- Facilitate multi-agency engagement for improved outcomes
Key Responsibilities
Core Duties
- Conduct culturally sensitive holistic health assessments, focusing on vulnerable groups (asylum seekers, Gypsy/Travellers, refugees, homeless populations).
- Perform TB/bbv screening and follow national/local guidelines for treatment/contract tracing.
- GP registration assistance and mainstream health integration for clients.
- Nurse-led prescribing in line with trust policies.
- Work across primary care, voluntary, and statutory services to address health inequalities.
- Teach, mentor, and act as a resource for other health professionals.

Support Services
- Maintain a weekly wound care clinic for street-based individuals.
- Sit in on MDTs, safeguarding meetings, and community outreach, advocating for trust values.
- Utilise interpretation services as needed.

- Maintain accurate digital records (IT systems, NMC guidelines).
- Follow Clinical Governance, Professional Practice, and Trust Policies.

Person Specification
Essential Requirements
✅ Registered Nurse (UK qualifications) ✅ Recent autonomous clinical decision-making experience ✅ Demonstrated teaching/training skills ✅ Government legislation knowledge in specialist areas (e.g., asylum seekers, tuberculosis) ✅ Understands health disparities (BME communities, Traveller groups, homeless populations) ✅ Valid driving licence (reasonable adjustments for disabilities)
Desirable Skills
🔹 Primary Care experience relevant to marginalised groups 🔹 Experience of working with interpreters and multilingual communities 🔹 TB screening & communicable disease management specialisation 🔹 Nurse Prescribing (recorded experience) 🔹 Mental health, sexual health, or related advanced nursing qualifications
