Transcend Specialist Nurse

Transcend is a newly commissioned service established to explore how Gender Incongruence can be best supported and treated within a Primary Care setting. We are seeking a Specialist Nurse to help deliver this care. The postholder will work closely with our MDT, local GPs, relevant partner agencies, and of course directly with service users.

This is an exciting opportunity to contribute to the development of not only a new service, but a new model of care for a disadvantaged and underserved service user group. The role will involve travel across the locality, therefore being a car user is essential.

Responsibilities

  • Provide direct clinical nursing care to the service users of Transcend.
  • This will include both direct and indirect interventions to support individuals accessing the service.
  • The ability to work autonomously, as well as part of a wider multidisciplinary team, is essential.
  • Manage a caseload of patients and support them throughout their journey with the service.
